The personal information members provide to us is collected solely for the purpose of providing members with PROVIDERS services and benefits. This information is protected by PROVIDERS by maintaining physical, electronic and procedural safeguards.

Personal information we collect about you comes from one or more following sources: paper application, faxed application, web application, electronic file transfer, or online enrollment via API. Personal information collected includes, but is not limited to: name, address, city, state, zip, telephone numbers, social security number, date of birth, gender, bank account information, credit card information, or any information gathered during a call with our Member Services or Provider Services departments.

We may disclose any or all of this information based on business need only, to our affiliates, service providers, third party service providers, provider networks and other suppliers of products and services.

Sharing Information with Affiliates and Service Providers

PROVIDERS will share member information with its affiliates, related companies and service providers to the extent it is necessary to provide and manage benefits to the member. Unless otherwise permitted by law, PROVIDERS will not share with these parties any information it collects that is not needed by these parties to provide the benefits to the member. These organizations are not authorized to use member information for any purposes other than for PROVIDERS business. All affiliates and service providers are required to conform to our privacy standards and allow us to audit them for compliance.

Sharing Information with Non-Affiliated Third Parties or Service Providers

PROVIDERS will not share, sell or otherwise disclose member information to any non-affiliated third parties or service providers. This includes third-parties and service providers not directly related to providing and managing benefits to the member.

Keeping our member information secure and using it only as needed for business reasons to provide services to our members is a top priority. We will safeguard, according to strict standards of security and confidentiality, any information provided to us by our members. We will permit only authorized employees who are trained in the proper handling of member information to have access to that information.
